Membership forms

The application form for Primary (Professional) and Associate membership of the Society, with guidance on completing it, the documents to attach and where to send it.

Form A is used for Primary and Associate membership. Students apply on Form B with a certificate from their Head of Department; write to the Secretariat for a copy.

What the form asks for

Print the form, complete it in block letters and affix a photograph.

  • The type of membership: Professional (Primary) or Associate
  • Your name, professional affiliation and designation
  • An official address for correspondence, email and telephone
  • Past appointments and an alternate postal address
  • Degree or diploma, current designation and institution
  • Particulars of your specialisation in cognitive neuroscience: clinical, research, teaching or technology development
  • Publications, on separate sheets
  • Special (core) clinical or research interests
  • Details of the subscription: amount, cheque or draft number, bank and date
  • A preferred mailing address, with city, state and PIN code
  • Your signature and the date
  • The recommendation of an existing member, with their membership number and signature

Declaration and documents

The declaration

By signing, you certify that the particulars are correct, state your interest in the objectives of CNSI and undertake to abide by the rules of the Society while you are a member. You also confirm that copies of your professional certificates are attached.

Documents to attach

  • Your current professional affiliation
  • Degree certificates, and certificates of clinical and research work in cognitive neuroscience
  • Your eligibility degree, a synopsis of research carried out and published papers
  • For Student Membership, a certificate from your Head of Department

Subscription and payment

Fees
Primary: ₹20,000 for life or ₹3,500 a year. Associate: ₹3,500 for life or ₹2,000 a year. Student: ₹1,000 for two years. Find your subscription.
How to pay
By cheque or demand draft in favour of “Cognitive Neuroscience Society of India”, payable at Bangalore. Add ₹85 bank charges for outstation cheques.

Receipts
Every payment is receipted by the Treasurer.

What happens next
The General Secretary approves eligible, nominated applications and the Executive Committee ratifies them by majority vote. New members receive a copy of the Bye-laws.
